Position summary

The case worker, Personalised Support Services is responsible for providing direct support to people residing in social and community housing who experience mental illness and whose tenancies are at risk. The primary role is to assist people to sustain their tenancies. The case worker will operate from a recovery model and offer a range of activities including but not limited to development of an individualised support plan, referrals, coaching and life skill development, community access, self care, developing relationships, managing money and tenancies. The case worker will also develop networks and referral linkages with Qld Health, Department of Housing and Public Works, Social and Community Housing providers.

The case worker is accountable to the Coordinator Mental Health Programs and works closely with Anglicare Central Queensland Community Housing Programs and other Anglicare Central Queensland Mental Health Programs. The case worker will also communicate regularly with local/ centre based Anglicare Central Queensland Management and work within a multi program and team environment.

Homelessness and Emergency Relief

Salary:                      $34.54 per hour.
Casual Fixed Term Contract 3 months – 48hrs per fortnigh

Position summary

The aim of the position is to provide case management support to people who are homeless, at imminent risk of homelessness, housed but at risk of homelessness via mobile support. Case management support is provided to people either in their home, in temporary living situations, in supported housing, in a community setting, or in a public space. The case manager will work closely with participants to develop, action and review case management plans to meet their individual needs, with the purpose of addressing any issues that may prevent them from obtaining and/or maintaining suitable, sustainable accommodation in the community.  This will include (but is not limited to) providing practical and personal support, crisis budgeting, information, advice and community referrals. Case management is approached from a strengths based perspective and will involve working across the Homelessness service including Emergency Relief programs as required.

Position summary

The Community Intervention Practitioner will work from a strengths based framework with families in a supportive and therapeutic role to assist families and individuals to make necessary and significant changes in their lives, which give hope, meaning and ensure safety.

The Community Intervention Practitioner will also facilitate and/ or co-facilitate educational or therapeutic groups that develop the individual or families ability to care for children and family members (e.g. parenting programs/ self development programs).

The Community Intervention Practitioner may be requested to participate in relevant interagency, support groups, reference groups that enhance the communities’ capacity to provide quality care as requested and approved by the Coordinator.
